AND CONDITIONS: Purchases must be made from Polyline between January
16, 2012 and March 31, 2012. Offer expires March 31, 2012. Rebate form
must be post marked by April 15, 2012. For rebate delivery, please allow
8-10 weeks from postmark deadline and following receipt of properly
completed rebate submission. Submissions must include 1) completed rebate
form with serial number from product box, 2) copy of final invoice and 3)
box UPC label. Please mail completed submission to the provided address.
Limit one rebate per household to end users (not for resale). Thirty-day
free trial programs are void upon participation in rebate program.
Delivery to P.O. Boxes is not accepted. Offer is not transferable. Primera
reserves the right to request additional information to validate claim,
making it subject to U.S. postal regulations. Offer void where prohibited,
taxed or restricted by law. Primera is not responsible for claims lost,
damaged or delayed in transit. Submitted materials become Primera
property. Products for which rebate is claimed may not be returned or
exchanged for non-eligible products. Offer valid only in the United States
